On
your first visit to our office you will be asked for basic information to
establish your medical record and business account. The information we ask
for is vital to your care. We ask patients to arrive 15 minutes prior
to their appointment so that they can complete the required information.
Please bring your current insurance information and notify our office of any
changes in name address telephone numbers marital status or place of
employment. You will be asked to present to us a form of identification to verify
your current address.
If
you should need a physician at a time when the office is closed please call
our answering service at (804)764-7734 and they will contact the
on-call physician. If the physician does not return your call within
15 minutes place your call again.
For
a life-threatening emergency,
chest pain or stroke symptoms dial 911.
Our patients are seen by appointments. We make every effort to keep our appointment schedule. Please understand that not all patients require the same amount of time with the physician and that emergencies do occur so some delays are unavoidable. If you are unable to keep your appointment please notify the office as soon as possible so that we may use this time for another patient. There is a $25 charge for missed appointments that are not cancelled ahead of time.
Our
charges for services - including office visits and procedures are based
upon the severity and complexity of your injury or illness as well as the time
spent treating you. Our staff will be glad to discuss our fees with you.
Please don't hesitate to inquire about the charges for our services. Payment
is due at the time of service if you are not covered by an insurance plan.
For your convenience we offer the following payment options:

Payment
in full.
·
Payment
of the portion your insurance will not cover on the day the service is provided
such as your co-payment or deductible.
·
Payment
of your balance in full upon receipt of your statement.
·
We
accept most major credit cards cash personal checks and bank debit cards
If
your visit is covered by health insurance we can file your insurance claim
if you bring your plans membership card. You must bring your current
insurance card for every visit. Please be reminded that your coverage
is a contract between you and your insurance company and that the ultimate responsibility
for payment to New Market Medical Center lies with the patient. Co-payments If
your policy involves a co-payment the amount of co-payment must be made at
the time of service or you will need to reschedule your appointment.
We
accept assignment of benefits from Medicare/Medicaid and HMO's. Insurance claim
forms will be filed for you with these carriers. It is the patient's
responsibility to pay any deductible amounts co-insurance or any other balance
left unpaid by your insurance company Medicare patients are responsible for the
yearly deductible and 20% of the approved amount only.
As
a courtesy to our patients we will file worker's compensation claims. However if
the claim is denied unsettled or unpaid within 60 days we will request that you
file a personal health insurance claim or pay the charges in full. You should
always notify your employer if there is any delay or problem in resolving your
worker's comp. claim.
If
you have been seen in the office recently, some prescriptions can be renewed by phone. When you call have your pharmacist's
phone number available. For most new prescriptions you will be asked to come in
for an office visit. Prescription requests received by 12 noon will be available
the same day. Request received after 12 noon will be filled the next day unless
it is an emergency. It is our policy to refill controlled substances only during
regular office hours.
Referrals
are processed on:· Monday through Friday 9 a.m. to 2 p.m.
The
physicians are required to approve all referrals. Please allow 3 to 5 working days for non-urgent referrals.
Same day referrals will not be approved if prior authorization was not obtained.
Emergencies will be processed within 24 to 48 hours. If you schedule your own
appointment please contact our referral coordinator with your appointment
information. Failure
to obtain referral authorization may result in your having to pay for the office
visit.
